"The Health Revolution"
A test was devised based on the symptoms exhibited in ADD/ADHD by a psychologist at the University of California - called the SNAP IV Rating Scale. It is based on the questions below, and the answers run from 1 (not applicable) to 5 (Yes, my kid all the time).
It may be helpful to copy this test, paste it into Notepad, and print it out, then mark the scores alongside the questions.
SNAP IV Rating Scale Questions
Often fails to give close attention to details or makes careless mistakes in schoolwork, work, or other activities.
Often has difficulty sustaining attention in tasks or play activities.
Often does not seem to listen when spoken to directly.
Often does not follow through on instructions and fails to finish schoolwork, chores or duties in the workplace (not due to oppositional behavior or failure to understand instructions).
Often has difficulty organizing tasks and activities.
Often avoids, dislikes, or has difficulties engaging in tasks.
Often loses things necessary for tasks or activities (e.g., toys, school assignments, pencils, books, or tools).
Is often easilty distracted by extraneous stimuli.
Often forgetful in daily activities.
Often fidgets with hands or feet or squirms in seat.
Often leaves seat in classroom or in other situations in which remaining seated is expected.
Often runs about or climbs excessively in situations where it is inappropriate (in adolescents or adults, may be limited to subjective feelings of restlessness).
Often has difficulty playing or engaging in leisure activities quietly.
Is always "on the go" or often acts if "driven by a motor".
Often talks excessively.
Often blurts out answers to questions before the questions have been completed.
Often has difficulty awaiting turn.
Often interrupts or intrudes on others (e.g. butts into conversations or games).
Often stares into space and reports daydreaming.
Often appears to be low in energy level, sluggish, or drowsy.
Often appears to be apathetic or unmotivated to engage in goal directed activities.
Often engages in physically dangerous activities without considering possible consequences.
Often shifts from one uncompleted activity to another.
Often fails to finish things he or she starts.
Has difficulty concentrating on school work or other tasks requiring sustained attention.
Has difficulty sticking to a play activity.
Frequently calls out in class or in other situations when silence is expected.
Needs a lot of supervision.
Moves about excessively (e.g., even during sleep at home or during quiet time at school)
Often acts before thinking.
Often loses temper.
Often argues with adults.
Often actively defies or refuses adult requests or rules.
Often deliberately does things that annoy other people.
Often blames others for his or her mistakes or misbehavior.
Often touchy or easily annoyed by others.
Is often angry and resentful.
Is often spiteful or vindictive.
Often swears or uses obscene language.
Often manifests provocative behavior.
Often shows excessive stubbornness.
